Great water sports. Activities/fairs/etc nearly every weekend. Very diverse population.

The Best Aspects….

Small command offers "family" atmosphere to the clinic. Corpsmen are wonderful and very helpful.  Hours are reasonable, great for family life and "other" interests.

The Worst Aspects…

No inpt care, unfortunately you have to give up responsibility of your patients should they need inpt care. (including no newborn inpt care).  Isolated duty station.  MOOD call includes calls about adult patiemts, Sat clinic (0800-1200) is acute care clinic, including adult patients.

Rural - farms. low cost of living for CA and within 3 hrs of SF, LA, beach and the mountain, but also the worst air quality in CA. good shopping is 40 minutes away in fresno. few good restaurants in town, but fresno again is not too far.

Great general peds experience with help not too far away. slower pace of living, great for families, awful for single people, unless you want to date a pilot, a farmer or a dairy worker.

Pretty affordable housing; I haven't heard of too many people having problems. Lots more activities during the summer months including beaches, outdoor activities. Not much exciting nightlife. Good place for families, probably not ideal for young single people.

Local Attractions:

Mystic CT (5 miles); New Haven (about 45 miles); Newport RI (1 hr drive).  Providence RI (1 1/4 hr drive); New York City and Boston MA (2 hour drive )

Good variety of patients, affiliation with local community hospital that allows us to take care of our own newborns and inpatients.

Can have some tough winters; not much night life.

Call is shared between the Navy pediatricians and the Air Force pediatricians. Call averages 4-6 per month but is by pager. Currently pediatricians have the next day off after call (nice bonus when you night is light).

Delivery Service:

Pediatricians attend deliveries of infants greater than 34 weeks depending on delivery risk. There is a busy NICU at USNH Okinawa which handles the level 2,3 babies.

Inpatient Service:

We do have a general ward for admitting pediatric patients as well as an experienced ICU nursing staff which can easily handle any critical care peds patients.

Outpatient Service:

The general pediatricians probably see 250 patients per month. It is a mix of well babies and sick children. Overseas screening is supposed to screen out really complicated children, but some still exist over here. We have a busy Asthma clinic and education initiative as well.

Deployment:

Not likely. Pediatricians do not deploy on platforms here.

Residents:

There is a Japanese Physician Intern Program.

Referral Centers:

The NICU is of course right in the hospital. The pediatricians handle all PICU patients in the ICU. Patients can be airevaced to Trippler Army Medical Center in Hawaii.

Community:

Housing is available both on and off base. Okinawa is a great place to live if you like to travel. You can go easily from here to Japan, China, Korea, Thailand, Singapore, Hong Kong, Australia, New Zealand.  There is a lot of good shopping especially furniture, oriental rugs etc.  This is a great place to come if you enjoy scuba diving as well as some of the best diving is available here.   This is a good safe community for raising children as well.

Local Attractions:

The island is a 2 hour plane flight from Tokyo and Hong Kong. On island there are castle ruins, underground naval museums from WWII, ocean expo parks, hiking, diving, pottery and glass artisans.

The Best Aspects….

My pediatric department -- we are a great family and I enjoy the sense of community and small practice. The TRAVEL -- we have already been to Australia, Hong Kong (twice), Singapore, Tokyo Disney and the city, Bangkok.

The Worst Aspects…

The distance from family and friends in the states.

Comments:

Anyone who has served here cannot wait to return. Once you buy a home and rent it out while gone, the economics get much better. There are very few negative aspects to living and working here. I put down roots in 1990 and plan to remain here permanently. NMCSD is a GREAT place to serve.

Community:

Kitsap Peninsula (Kitsap County) is the relatively more rural / suburban 'West Puget Sound' neighbor of Seattle's urban environment and the Olympic Peninsula / Mountains to the West, the Cascade Range to the East, and the Strait of Juan de Fuca / Washington.  San Juan Island, and the U.S. / Canadian border to the North. Easy access to all by car and / or by ferry. Life (and traffic) are delightfully slower / less hectic in NHB's little corner of the world. It rains in the winter. Temperatures are moderate year-round. Housing available Poulsbo, Silverdale, Bremerton, Port Orchard, Gig Harbor and surrounding area.

There is not a great deal to do here unless you like the outdoors. There is a very large mall about 1 hour away and there is a K-Mart and Walmart 10 minutes away. For most types of entertainment, one can go to San Juan (1 hour) and there are a great number of restaurants and things to do there.

The hospital is really great to work in and the weather is beautiful.

The surrounding community can be both wonderful and frustrating. The lack of things to do can also be difficult.

Overall, this has been a great tour. There is enough to keep busy, but not so much that one can't spend time with family and enjoy other things.

Delivery Service:

Yes.  400 deliveries per year.  Responsible for being at c-sections, VBACs and any other high risk deliveries.

Inpatient Service:

25 bed general med/surg/peds ward. Typical inpatient admissions include respiratory things (asthma, bronchiolitis), r/o sepsis, VGE with dehydration, pyelonephritis.  No ICU facilities.

Outpatient Service:

Average is 17-20 clinic patients per day.  Cut shorter on weeks that you are on call (starts at 0900 rather than 0730) so that inpatient duties can be taken care of. There is a LARGE amount of pediatric psychiatry here. The pediatricians act as the subspecialists in most areas llot of times (with phone support from the actual specialist) because of location. This is an EFMP level 5 area (we take all patients) since Madigan is only 2-3 hours away --  problematic but you get exposed to wide range of kids.

All subspecialists/NICU/PICU are either from Madigan or Childrens in Seattle (3 hours and 2 hours respectively).

Community:

Small - no shopping on the island except for exchange and everyday grocery. There is a small movie theater in town. Plenty to do as far as outdoor recreation with hiking, skiing, crabbing, boating, etc. Nearest mall is about 45 minutes away off the island. Major mall and entertainment is in Seattle or Vancouver, both about 2 hrs away.

Local Attractions:

Seattle, Vancouver, San Juan Islands.

Yes. Deliver approx 100 babies/month. Present for all C-sec during work week and significant deliveries after hours. Generally do not deliver here < 35-36 wks.

Inpatient Service:

Yes. Average census of 1-2 pts during summer, 3-5 during winter.

Outpatient Service:

Typical load is 23-26 per day, at 4/hr. "Teach" doc sees 3/hr. Typical general peds population. Also see full spectrum of EFMP categories due to access to subspecialties and inpatient services.

Deployment:

2 of the 5 billets are on platform (1 USMC, 1 fleet hosp)

Residents:

FP residency in-house, intern on inpt service. 1-2 from local peds residency see cont clinic here.

Referral Centers:

Local Childrens Hospital in town with PICU and access to multispecialty clinic. Specialists very accessible.  NICU is 15-to-20-min drive away, has transport team.

Community:

"Up-and-coming" town, beginning to bring in broader spectrum of recreation and cultural events. Housing is SUPER cheap. Lots of new construction/planned communities to buy into. Locals are very into their football (college and NFL).

Local Attractions:

St Augustine, historic district with really good restaurants/clubs is 20 min south.
Just over 2 hours from Orlando.

The Best Aspects…

Great opportunity to do full-spectrum pediatrics; good variety of patients; lots of opportunities to teach; good place to raise kids

The Worst Aspects…

Hard to stand out as an O-4. Not a very metropolitan city; some racial tension in parts of town; big variation in quality of schools

In general, it is home call with a pager, as NHCL has a Family Practice Residency Program.

Delivery Service:

Inpatient Service:

Nursery, a "Level II" Nursery, and a general inpatient ward.

Outpatient Service:

The Pediatric Clinic will see 25 patients a day. Generally, the patient population is well, but Camp Lejeune is a Level IV EFMP duty station for the Marine Corps, so we do have challenges.

Deployment:

Likely -- we have a Fleet Hospital, the 2nd and 3rd FSSG that deploy, and Pediatricians are part of the platform

Referral Centers:

There are a number of Network areas that children are referred for specialty care. For inpatient referals/transfers, they will depend on the general availability/bed space at those centers.

Community:

There has been marked changes to the area. There are a number of new stores to include a Target, Old Navy, Pier One, and more. There are a number of different restaurants to choose from. There is also Onslow Beach (on base), Emerald Isle, Snead's Ferry, and Topsail -- all very nice beaches

Local Attractions:

Willmington -- home of the USS North Carolina. Swansboro for quaint waterfront village, shops, and antiques. Raleigh is about 2.5 hours west.

The Best Aspects…

working with the Marine Corps and thier families. These are people that are the "tip of the spear" and really appreciate the medical support. Further there is a sense of honor to care for the loved ones of our Nation's Finest.

The Worst Aspects…

working wih the Marine Corps...they, at times have a limited understanding of the complexities of the clinical and administrative aspects of medicine.

Comments:

NHCL has successfully graduated it first set of Family Practice Residents in 2005. There are now a complete number of residents for each level -- 6 Interns, 6 PGY-2, and 6 PGY-3. Thus, the hospital attendings now have a little more luxury in that we can better rely on the senior residents on-call. HOWEVER, there is still some evolutions to their schedules and expect changes for the next three to four years (but this is an aspect that can be exciting).  Updated 7/2005
