Type of call:

Phone call only,

Delivery Service:

None

Inpatient Service:

None

Outpatient Service:

Templates have 22 to 25 patients per day, 15 to 20 min appts. Fairly flexible depending on provider preferences. Primarily healthy children, about 10% special needs patients.

Deployment:

possible, not probable

Residents:

No

Referral Centers:

Oklahoma Children's Hospital is 10 minutes from the base, with full service NICU, PICU, ER, specialty depts. Three other NICUs in town, one other PICU.

Community:

OKC is a moderately large city, several nice restaurants downtown and in surrounding suburbs. Areas around the base have more older housing. Many officers live in Edmond or Norman OK. both about 25 minute drive from base, more newer housing and better school systems. There is some officer housing on base, but it is small duplex units.

Local Attractions:

OKC, Dallas is just a 3 hour drive. Ozarks in Missouri and Arkansas about 3 to 4 hour drive.

The Best Aspects….

Ideal assignment for pediatrician who just wants to do outpatient. There is no ward, nursery or ER. We do have full service Pharmacy, Lab, Radiology in house, as well as Orthopedics, Surgery, and Gyn (not OB).

The Worst Aspects…

Not ideal for someone who wants to keep up acute care, nursery, delivery room skills. OKC is rather isolated geographically for people from East or West coast areas.

# Pediatricians:

Length of Tour:

2 years

Frequency is call?

Medical Officer of the Day (MOD) approximately 5x/month.

Type of call:

Phone coverage of 11,000 F.P., IM, Flight Med, Women's Health, and pediatric patients. Referral ER and "doc in box" clinic at medical center downtown. No separate peds floor nor PICU/NICU.

Delivery Service:

No

Inpatient Service:

No

Outpatient Service:

20 - 30 (season dependent): 50-60 higher need patients - handful of diabetics, multiple atopic individuals with moderate/severe asthma, some syndromic children, and normal assortment of mildly disabled individuals. acuity is generally light.

Deployment:

Unlikely

Residents:

No

Referral Centers:

Jacksonville 1.5 hours.  Tallahassee 1.5 hours.  Atlanta 3 hour.  Augusta 4 hours

Community:

small town deep south. housing is cheap but BAH decreased appropriately. nice communities with family-focused population.

Local Attractions:

recreation is limited for non-family activities; wild adventures animal and amusement park is nearby and great for families, malls and shopping is limited for true shoppers but is adequate for those of us who don't do it well, and there are plenty of eating establishments. there is one theatrical troupe in town that puts on performances quarterly (roughly). two movie theatres but only one true coffee shop and one shop in a bookstore (no starbucks!). valdosta state university in town. Tallahassee: not a bad town but haven't truely explored yet.  jacksonville: same as above.  st simon island: georgia coast and nice beaches. quaint and pleasant for even a day trip, better overnight.

The Best Aspects….

med group personnel - supportive leadership (mdg commander is a pediatrician) and decent comraderie.

The Worst Aspects…

limited for unmarried personnel (limited night life, metro areas outside of spontaneous night trips, high mobility for line officers so friendships intermittent at best)..flat surroundings..not conducive for nature adventures.

Comments:

we 'map-g'd' into dropping an active duty slot for a second pediatrician and instead planned on contracting out the position.  Uncertain as to status.

Length of Tour:

2-3 years

Frequency is call?

Every 5th week

Type of call:

Pager

Delivery Service:

None

Inpatient Service:

None

Outpatient Service:

About 25 patients per day. Mainly children 4 years old and younger. Older children are complicated (ADHD, ODD, Diabetes, Cystic Fibrosis, etc).

Deployment:

Minimal

Residents:

No residents

Referral Centers:

Minimal resources in the immediate area (30 minutes away). Some resources at Cottage Hospital in Santa Barbara (1 hour away). Main tertiary centers are in Los Angeles (3 hours away).

Community:

Anything good is about 30 minutes away. Housing is very expensive ($1450 per month for rent, $300,000+ for a descent home). Some outdoor recreation. Entertainment is lacking besides movies. Good shopping is at least an hour away (Santa Barbara).

Local Attractions:

Solvang, which is a small Danish community with good shops and restaurants is about 30 minutes away. Santa Barbara is about 70 miles away. San Loius Obispo, a small college town, is about 50 miles away.

The Best Aspects….

The weather is mild - 60's to 70's year long. No inpatient services or ER.

The Worst Aspects…

The hours average 60+ per week, which is a lot, considering there are no inpatient duties. The lack of another Pediatrician, NP, or PA makes it difficult to take some time off, since nobody can cover easily. PCO system isn't helpful, since we aren't manned for it. This population could easily support another Pediatrician.

Delivery Service:

Yes. 70-100 deliveries per month.  Very busy.  Frequent respiratory and cardiac complications that require transport

Inpatient Service:

Yes, MSU and general ICU available

Outpatient Service:

25-27 per day, EFMP referral center, CF, Diabetes, severe CP, CHD.  EFMP base

Community:

Emerald coast of florida.  Nice beach.  Fair to poor shopping.  Fair entertainment.  Outdoor water people would enjoy this area.

Local Attractions:

Pensacola, Panama City, Destin, Seaside

The Best Aspects….

Keeps inpt and delivery skills current as high delivery rate and EFMP status gives good outpt problem lists.
Nice area to live in as single or family.  I love the frequent high acuity of the patients that enable the ability to maintain skills with NRP, intubation, ventilators, central lines.

The Worst Aspects…

Education for FP residency, so frequent job title is as glorified second year teaching interns.  Frequent call with frequent busy nights.

Growing city of approx  500k.  Many new houses being built all the time; single-family homes about $170,000-200,000 in nice areas. School districts depend on where you live - better on north end of town. Recreation - plenty of outdoor activities - hiking, climbing, skiing. Shopping - okay malls; true shopping about 45-60 minutes away (outlet mall in Castle Rock and malls in Denver). Entertainment - low budget mainly (movies, etc.); fine arts in Denver. Restaurants are nothing in comparison to San Francisco or D.C. Very family oriented. Lots of churches of various Christian faiths. Very safe; low crime.  Nice mountain views.

Pikes Peak in Co Springs - highest mountain; beautiful views; can climb or drive to top. Denver - good size city, with usual museums, restaurants, etc. Skiing about 2-3 hours away (including Vail, etc.). Garden of the Gods in Co Springs - beautiful rock formation; great for hiking. The Air Force Academy - interesting to see; chapel is incredible; home football games all the time in the fall. Cripple Creek - gambling; about 30-45 minutes away.

Type of call:

Pager

Delivery Service:

None

Inpatient Service:

None

Outpatient Service:

NICU downtown in Rapid CIty; no PICU; closest major referral centers in Denver or Sioux Falls (6 hrs+ away)

Community:

Housing is expensive; outdoor recreation great (skiing, hiking, camping, biking, swimming, etc.)--cultural activities so-so, shopping fair to poor for selection

Local Attractions:

Badlands Natl Park, Custer state park, Black Hills National Forest, Mt. Rushmore, Deadwood, Crazy Horse Memorial

The Best Aspects….

Outpatient based practice, autonomy high under current administration, reasonable workload and empanelment, infrequent call

The Worst Aspects…

Solo slot (little opportunity for consultation with other peds folks), few peds subspecialists in immediate area,

Comments:

This is a great starter slot coming out of residency if you are interested in outpatient general pediatrics. You will learn a lot without too much stress and no inpatient or nursery responsibilities. However, if you like inpatient or nursery this is not the assignment for you. Family life is good here due to regular hours and no weekend clinics, and call is taken from home by phone. Overall a very nice assignment in a beautiful outdoors-oriented area. Not the place for someone who likes big city life, however. I will be leaving in summer or fall 2004, so a slot will be opening then.

Community:

Bitburg/Spangdahlem is located on the very western side of Germany, with very close access to Luxembourg, Belgium, the Netherlands, and France.  There are some very good cheap airlines that fly to Spain, Italy and the UK.  Officer housing is limited, so most officer families live on the economy.  Most German nationals speak a little english and are very friendly.  “The base is nothing to talk about, and the housing is the worst I have seen, but I live off base in a little village.  The base is in the middle of the country.  There is not much here, but there is a lot within an hours drive.”

Local Attractions:

Trier, Koln, Luxemburg, Aachen

The Best Aspects….

The travel!

The Worst Aspects…

Frequent call, the weather.

Comments:

Being overseas in any location is one of the greatest perks of the military.

Length of Tour:

Frequency is call?

Q4, 1 weekend/ month.  Average is 6-8 times per month

Type of call:

Pager.  No PCM pager (no mommy calls).

Delivery Service:

We have 35-50 deliveries/ month.  None < 36 weeks.

Inpatient Service:

Ward that has adults and kids.  1-2 peds admissions/ week; more in winter.  Staff has little peds experience.

Outpatient Service:

25 patients/ day.  Lots of asthma, allergies, and chronic OM. Also a fair number of ADHD, developmental delay, and behavior problems. Families are needier in terms of visits due to frequent single parenting due to deployment

Deployment:

Unlikely to fair - more likely to be humanitarian.

Residents:

No.

Referral Centers:

Germany bases and British doctors.  NICU and PICU are in Cambridge (25 miles) or London (2 hrs away)

Community:

It is best to live in a British village so you get the full experience of England. The surrounding community is great.  There are not a lot of malls or movie theaters. We are definitely in the country. For families with teenagers, this may not be the place because there are not a lot of activities for that age group. Stores in the area close around 5pm.  Shopping & food are expensive. Can get just about anything you need on base or through the local community.  Houses here are smaller than we are used to. Small closets, terrible showers, lovely gardens.

Local Attractions:

London, Cambridge, Bury St. Edmunds, Lavenham, Norwich, Ipswich.  Excellent theater.  Travel to Paris [1 hr by train], Wales, Scotland and nearby Europe.

The Best Aspects….

England and traveling, 4 pediatricians and 1 NP.

The Worst Aspects…

long days, many tasks other than clinic, being away from family.  The patient load - high with a very needy community. Very high op tempo base with PRP program. High average visits/year, a good 10% are high utilizers. Poorly staffed, one of our docs has had no nurse for 2 years, despite leading a PCO team.  The weather is pretty bad and makes allergy/asthma season year round.  “Exceedingly demanding patient population, very high rate of deployments….”   “This area does wear a person down.”  “High Stress Environment.”
